Wraparound is a process that brings people together from both family and community to work toward a common goal for the benefit of the child and their family.

Who May be Eligible?

  • Children who live in Southeastern CT
  • Children who are birth to 5 years of age
  • Children who may be at risk of out of home placement or already placed
  • Children that have a diagnosed or emerging mental health condition
  • Children currently being served by multiple agencies/systems

What to Expect in Making a Referral

Building Blocks will be working with families who qualify to receive intensive home and center-based services.

To begin the referral process, contact 860 434 5170

The Intake Coordinator will screen the child and family situation and provide you with additional information and a Referral Form (in English or Spanish).  At this time, a signed consent by the legal guardian is needed for the purpose of intake only.  Each referral is given individual attention and consideration for the program.

Families selected for the program will be contacted.  Families that are not enrolled in Building Blocks will be provided with referrals to other supports in the area.  An ECMHP and FP team will be available for each family.  

Building Blocks operates in accordance with the Health Insurance Portability and Accountability Act (HIPPA)
