What to Expect
in Making a Referral
Building Blocks will be working with
families who qualify to receive intensive home and center-based
services.
To
begin the referral process, contact 860 434 5170
The Intake Coordinator will screen
the child and family situation and provide you with additional
information and a Referral Form (in English
or Spanish).
At this time, a signed consent by the legal guardian is needed
for the purpose of intake only. Each referral is given individual
attention and consideration for the program.
Families selected for the program
will be contacted. Families that are not enrolled in Building
Blocks will be provided with referrals to other supports in the
area. An ECMHP and FP team will be available for each family.
Building Blocks operates in accordance
with the Health
Insurance Portability and Accountability Act (HIPPA)
